I certify that my dog is in good health and has not shown signs of contagious illness within the past 30 days; is current on the vaccinations required by Dog-ology, including rabies, DHPP (distemper/parvo), bordetella, canine influenza, and leptospirosis ; and is on a veterinarian-recommended flea and tick preventative. I will provide current vaccination records before my dog’s first visit and keep them up to date. If my dog requires medication during care, I will provide written instructions and authorize Dog-ology staff to administer it as directed.

6. Assumption of Risk
I understand that dogs interacting with people and other dogs — in group play, socialization, training, and routine handling — involves inherent risks that exist even with attentive, professional supervision. These risks include scratches, nips, bites, sprains and other injuries, and the transmission of communicable illness (such as kennel cough). I knowingly and voluntarily accept these risks on behalf of myself and my dog.
I also understand that time outdoors carries risks that cannot be fully eliminated — including insect stings, ticks, and encounters with wildlife — and that dogs may eat things they should not, such as sticks, rocks, mulch, plants, mushrooms, or other objects found outside. Dog-ology supervises closely and takes reasonable precautions, but cannot guarantee that a dog will never contact wildlife or swallow something harmful. I accept these risks on behalf of my dog.

9. Responsibility for Your Dog
I am responsible, to the extent provided by law, for injuries or damages caused by my dog to people, other animals, or property while in Dog-ology’s care, and I agree to reimburse reasonable related costs, including veterinary or medical expenses.
11. Fees, Late Pickup & Cancellations
Pricing is quoted individually for each dog and service and confirmed with me before services begin. Payment is due upon receipt of the invoice. Daycare is offered as a half day (up to 4 hours) or a full day (up to 8 hours); longer days are quoted individually. Boarding check-in is from 4:00 PM and check-out is by 11:00 AM. Late pickups may incur additional charges. Cancellations require at least 48 hours’ notice; late cancellations or no-shows may be charged up to 50% of the quoted service.
12. Extended Stay
If my dog is not picked up within 3 days of the scheduled pickup and Dog-ology cannot reach me or my emergency contact after reasonable attempts, my dog may be deemed abandoned in accordance with applicable Maryland law and placed with a licensed shelter or rescue. I remain responsible for all accrued boarding and care costs.
13. Personal Items & Safety Discretion
Dog-ology is not responsible for loss of or damage to personal items left with my dog (leashes, bedding, toys, etc.). Dog-ology may separate dogs, modify activities, or decline or discontinue services at any time if it judges that necessary for the safety or wellbeing of any dog or person.
14. General
This Agreement is governed by Maryland law; any dispute will be resolved in the courts located in or serving Frederick County, Maryland. If any provision is found unenforceable, the rest remain in effect.
